Provider Look Up Help

Purpose

Users can find an active medicaid provider using several sets of criteria.

Search Criteria

By Name

• Search by the Provider's Last Name
  • You do not need to enter the entire provider's last name.
  • Enter as little or as much as you know. The match will look for last names that begin with the values you enter.
• Search by Institution/Office Name
  • Use the dropdown box to get a list of all providers at a particular Institution or Office.

By Location

• Search by the County Name
  • Search by a County in Vermont
  • Choose out of state to search the surrounding areas of Vermont
• Search by Zip Code
  • Enter a five digit zip code
• Search by City
  • Use the dropdown box to select a city name as the search criteria
  • Since some cities in Vermont have duplicate names in other states, the state is included in the search.
• We suggest not using both Zip Code and City in the same search. The data is provided to us directly from providers. Sometimes a zip code has been associated to an incorrect city.

Refine your search

There are several check boxes that you can select to limit your search to certain criteria.

• Checking the "Are you looking for a Primary Care Physician who is enrolled in Primary Care Plus (PC+)?" box limits your search to just those providers who are enrolled in PC Plus and specialize in one or more of the following:
  • Family Practice
  • Geriatrics
  • Internal Medicine
  • Naturopathic Physician
  • Physician - General Practice
  • Pediatrics
• Use the Specialty dropdown to limit your search to a specific specialty.
• Checking the "Are you looking for a provider that is accepting new patients?" box limits your search to those providers who are currently accepting new patients and those providers whose new patients status is unknown.
• Checking the "Are you looking for a provider whose office is handicap accessible?" box limits your search to those providers whose office is fully handicap accessible, partially handicap accessible, and whose handicap accessibility is unknown.
• Checking the "Are there any special language requirements?" box activates the Language Selection dropdown box.
  • Limit your search by selecting a specific language.
  • Only languages that at least one provider has reported as spoken at their office are listed.
• Checking the "Are you looking for a provider of a specific gender?" box activates the Gender Selection dropdown box.
  • Limit your search by selecting a specific gender.
  • Only providers whose data shows them as the specific gender you selected, are listed as Both or whose gender is unknown, will be returned.
• Checking the "Are you looking for a provider who serves specific ages?" box activates the Age box.
  • Enter the of the individual looking for a provider.
  • All providers whose listed age range includes the age you entered will be returned.

Buttons

• Clicking the Find button intiates the search. You can also press the return key on your keyboard.
• Clicking the ResetForm button clears all selected search criteria from the form.
• Use the to increase the font size.
• Use the to decrease the font size.

• The Accreditation Association for Ambulatory Health Care links to a search engine operated by AAAHC for locating providers who have received accreditation certificates from AAAHC.
